The Process

There are two ways to get a laminated foam cushion: through insurance with a prescription, or by paying out of pocket.

Option 1:

Insurance-Covered

(MD Prescription Required)

Steps:

  1. Doctor prescribes the laminated foam cushion

  2. Supplier submits authorization to insurance

  3. Referral to a clinician or ATP (if you don’t plan to cut it yourself)

  4. Clinician or ATP cuts the cushion — or you perform the cutout yourself

Option 2:

Out of Pocket

(No Prescription)

 

Steps:

  1. Purchase the cushion directly from a recommended resource

  2. Referral to a clinician or ATP (optional)

  3. Clinician or ATP cuts the cushion — or you perform the cutout yourself
